Once you pass the test, you are eligible to receive a certification card. To receive your card, fill out this form, then print it out using your browser's FILE/PRINT button or the PRINT PAGE link below and mail to the following address with your check or Money Order in the amount of $109 for your certification card or $139 to receive both a certification card and a certificate.
